(Michelle) Polk and Jeffery Polk; two sisters, Mary (Arthur) Polk Brundidge, Brenda (Calvin) Polk Jackson; god brother, James Hodges; six grandchildren, Chaquria Willis, Ebony Sullivan, Terrence Johnson Jr., Courtney Johnson, Myracle Bussey, Terrence Bussey and a host of nieces, nephews, cousins and friends.
